Cancellations
We request that you provide adequate notice if there is a need to cancel or change appointments. We require 48 business hours advanced notice for cancellations. No shows or cancellations less than 48 business hours before the appointment will be charged the full visit amount. For example, a Monday appointment at 1:00pm would require advance cancellation before 1:00pm on the previous Thursday.
New Patient Consultations
Your first visit is considered a consultation and it will allow your psychiatrist to provide you with their clinical impressions and initial treatment plan. Any prescribed medications or lab orders will be sent electronically to your preferred location. An appropriate time frame for your next visit will be determined (usually within 2 weeks) and an appointment will be set in the office calendar.
Additional Services
- Out of pocket charges may be incurred for other additional services. This includes, but is not limited to, the following:
- Filling out legal forms for FMLA or disability. (Agave Psychiatry does not fill out forms for FMLA or disability until a patient has had at least 3 visits with their physician.) We are not disability providers and may refer patients to other physicians who specialize in these evaluations.
- Requests for emotional support animal letters.
- Requests for treatment summaries and/or discussing your treatment with other healthcare providers.
- Physician depositions or time in court.
- Prolonged phone calls in-between scheduled visits.
- Prior authorizations for brand-name medications.
- Specific prices available upon request.
